Kaeso 0.4.0

Kaeso 0.4.0 lets organizations add ChatGPT accounts as OpenAI provider keys, and Grok accounts as an experimental SpaceX provider key, so projects can draw on those plans instead of API accounts. Nothing in this release is breaking, so existing provider keys and integrations need no change. The gateway's ctutils dependency moves to 0.4.4 to fix a timing advisory in encryption and password hashing, so upgrading is recommended for every install.
